Claude Code loaded instructions missing from its inventory

We could trust our instruction inventory and send Claude Code into a run with a rule the list said did not exist. Claude Code could still load that instruction, which meant the inventory was incomplete. what the nightly check accepts The instruction and its row both exist: The instruction exists, but its row is missing: The row exists, but its instruction is missing: The row says dead without a stop marker: The row quotes text the instruction no longer has: The nightly check now records a failure whenever the inventory and the instruction files disagree.
