Codex's skill-status-gate marks missing skill files as tombstones

Ops log: The skills inventory told Codex 105 instruction files existed, while 123 were on disk. It was a snapshot, so new files had no row. Twenty files were missing from the inventory. We wired skill-status-gate.mjs into the nightly run, and it now fails when a file and its row disagree.
